It could also be that the name of the package changed:

libmythtv-0.20.1_0.i386                  0.20.1-161.el4         atrpms
libmythui-0.20.1_0.i386                  0.20.1-161.el4         atrpms
libmythupnp-0.20.1_0.i386                0.20.1-161.el4         atrpms

libmyth is installed and the updated package is called libmyth-0.20.1_0.
