There was another user over on ivtv-users having this same problem with the 
same setup I do.  As far as he's been able to track down, it's a problem with 
ivtv 0.7.0, kernel 2.6.17, and x86_64 [1].

I was able to revert to kernel 2.6.16 (found packages) and ivtv 0.6.1 
(manually compiled) and I was able to get everything working.

Another problem I've found is that when I use the separate -enc and -dec 
firmware packages I don't have any audio output, but I do with the combined 
one.
