I wonder whether you have a bad card. There haven't been any such
reports for quite some time. There were similar issues a year or two
bad, when the firmware loading wasn't as good as it is now.
I'd take that to the ivtv lists. Including your card's data (PCI ids,
model numbers etc, all that dmesg gives between the two ivtv start/end
markers.
